The Board of Pharmacy had emergency rules become effective on January 31, 2014 which increased the fees for the following applications. Any application received or downloaded prior to this effective date will have the old fees printed on them but if the application is received by mail after February 14, 2014, the increased fees will be charged. If you renew online the new fees will be charged as of the effective date of the rule.
